SERVICE PERSONALITY TEST – SELF ASSESSMENT FORM
This is a self assessment form that is one part of the package that we use to assess the Customer Service Footprint of Companies. The Form has a list of questions for you to answer on a scale of 1 – 5. After scoring yourself on each service, find the total for the list of dimensions.
WHO SHOULD COMPLETE THIS FORM
Ideally this form would be filled out by an Executive or Senior Manager of the company. This person will have the necessary background and overarching view of the company that will enable them to answer these questions. This form can also be filled out by Departmental Managers, Managers or other individuals who may also have this information, or who may want to fill out the form based on their knowledge of their department.
SCORING LEGEND
Once you complete the form, tally your scores, and find your overall total. You can then determine which category your company belongs to by matching your score to the appropriate section of the Scoring Legend. Companies can score anywhere between “Careless with Customers” to “Customer Centric” . Based on your results, this will give you an idea of where your company stands with respect to its service personality.
